The (National Pest Management Association) NPMA-33 Form or “Wood Destroying Insect Inspection Report” is at times one of the requirements of VA, HUD, and FHA loans. In California, we have the standard report form that is issued by the Structural Pest Control Board, and at times both forms are requested by the lender.

In any of the scenarios your licensed termite control inspector should complete and sign this form. The forms can be filled after an inspection of the property is completed. This has to be done by a licensed inspector and company that has no interest in the property or on the transaction. What this will state is if there are any problems related to wood destroying insects or organisms. But what are these W.D.O.s? They are pests that attack your structure and include Termites, Wood Rot, Carpenter Bees, Wood Boring Beetles, among others.

One important thing to note is that the NPMA-33 form was updated in July 2019 by the National Pest Management Association. There were changes completed in the verbiage like changing “or defects” with: “or wood destroying insect damage”. It as well removed the section that asks if the is evidence of past treatment, among other important changes. You can see all of the changes that were done on this Letter Template from the NPMA.

Some of the information that needs to be filled out includes:

• Section I. General Information
  • This includes the termite inspector and company information and signature. It will as well include the address of the property that this form is being completed for.
• Section II. Inspection Findings
  • It will include any findings that were found during the inspection. This can be either visible insects (either dead or alive) and any damage that has been created by any wood destroying insect.
• Section III. Recommendations
  • In this section, your termite inspector will provide its recommendations to resolve any issue that was found
• Section IV. Obstructions and Inaccessible Areas
  • Any areas that are not accessible during the inspection will be listed in this section. They vary from fixed ceiling to cluttered spaces, cabinets or shelving, also things like furniture or storage items, among others.
• Section V. Additional Comments and Attachments
  • In this section, your inspector can include any additional comments that he believes should be considered. It will include information as well about any attachments, for example, The report number of the one completed using the state form.
• Signature of Seller(s) or Owner(s) if refinancing
• Signature of Buyer

This information, along with other details that might be thought to be of interest will be listed on the form by your licensed inspection.
